DESCRIPTION OF PLACEMENT:
This ST post provides broad experience in general adult psychiatry
at ST4-6 competency level. The post is essentially community based
with CMHTs in the Carlisle and Eden area. Working with two CMHTs
allows experience to be gained in working in both and urban and
rural environments, and with different teams.

ROLE OF TRAINEE:
The ST4 will work primarily in community settings working closely
with the CMHT and consultant and will undertake assessment and treatment
of patients with a wide range of psychiatric disorders. They will
manage their own case load with the CMHT participating in CPA, multiagency
risk meetings and liaising with other services, seeking consultant
advice as appropriate. They will liaise closely with more specialist
teams such as AOT, CRHT, inpatient and EIP as the need arises and
there will be opportunities to do special interest sessions with
these teams and others if requested. They will be involved in MHA
work with the CMHTs that they work with.
Consultants will be operating in a “Chambers model”
with New Ways of Working in service delivery, and the trainee will
have the opportunity to get more experience with these innovative
models.
There are 5 sessions with Carlisle CMHT and 1 session with Eden
CMHT, including CPA clinics, assessments MDT meetings, risk management
meetings ( child protection, MAPPA, vulnerable adult).
As members of the multi disciplinary teams the trainees will be
expected to take advantage of the opportunity to learn how other
disciplines, including Nursing, Psychologists, Occupational Therapists
and Social Workers operate.
ON CALL COMMITMENT:
The on call commitment will be second on call as 1 in 10, for north
Cumbria, with responsibilities for General and Old Age Psychiatry
and support from the on call consultant if required.
TEACHING/ACADEMIC EXPERIENCE:
The ST will be expected to take part in teaching and appraising
Newcastle medical students placed with the CMHTs. They are expected
to be involved in teaching CT, GP VTS and foundation doctors both
through the academic programme, and with the acute trust and as
specifically arranged events for instance teaching interview skills
with the psychotherapy department. There will be opportunities for
multi-professional teaching and learning through the CMHTs.
AUDIT/REASEARCH OPPORTUNITIES:
Trainees would have the opportunity to do research with one of
the Cumbria consultants if they are interested. The Trust is currently
working with Lancaster University in research in adult psychiatry
(psychoeducation in Bipolar disorder). The educational supervisor
has published research in areas including suicide and staff stress.
MANAGEMENT TRAINING:
Management opportunities are both through shadowing senior managers
and getting involved in a management project ( with appropriate
supervision) and through representing junior doctors (e.g. on medical
education committee) and doctors ( audit and governance meetings).
The trainee should discuss with their supervisor appropriate management
opportunities to meet their learning needs.
ADDITIONAL EXPERIENCE AVAILABLE:
This post offers a rare opportunity to practice psychiatry in
rural setting and contrast it with urban psychiatry.

IMPORTANT ADDITIONAL DETAILS:
Post is CMHT.
However it can become a core post if some of the sessions include
inpatient work.
For instance a special interest session could be in PICU or Rehabilitation
and one of the other community sessions could be converted to the
same inpatient work.
There is an ST1-3 in the team.
