StR/ SpR POST DETAILS 2009-2010
DETAILS OF PLACEMENT:
This is an ST 4-6 placement in Community General Adult Psychiatry
working with the Central Manchester Crisis Resolution Home Treatment
(CRHT), Central Manchester West Sector Community Mental Health Team
(CMHT) and Manchester Homeless Mental Health Team.
DETAILS OF EXPERIENCE TRAINEE WILL GAIN AND MAIN DUTIES:
The post-holder will gain extensive experience in general adult
psychiatry working closely with Dr Jefferson across three community
based services; the Manchester Crisis Resolution Home Treatment
(CRHT), the Central Manchester West Sector Community Mental Health
Team (CMHT) and the Manchester Homeless Mental Health Team.
Central Manchester CRHT developed from “Home Options”
– in innovative service that has offered home based treatment
in central Manchester for the last 10 years. The service provides
intensive care in the community for care for general adult psychiatry
patients as an alternative to hospital. The post-holder will have
the opportunity to be involved in new patient assessments and domiciliary
reviews. As the post-holder gains experience during their attachment
they will be able to take on directing the treatment of individual
patients throughout complete episodes of care (at all times under
the direct supervision of Dr Jefferson).
Central Manchester West Sector CMHT serves the Hulme and Rusholme
areas of Central Manchester. Consultant workload for this team is
divided by GP practice with Dr Jefferson taking responsibility for
the Hulme area (approx ~17000pop). Hulme is a diverse and vibrant
inner-city area that has recently undergone a transformation with
extensive redevelopment and regeneration. West Sector CMHT is based
at the Kath Locke Centre, a multipurpose community resource centre
on Moss Lane East. The CMHT team works mainly with patients with
severe and enduring mental illness. The post-holder will be able
to attend weekly CMHT allocation meetings and will be able to see
new referrals either as out-patients (at the Rawnsley Building)
or on home visit together with CMHT staff. The post-holder will
not inherit any follow-up out-patients. All out-patients will be
discussed in regular clinical supervision with Dr Jefferson, and
at the end of the attachment those needing on-going follow-up will
be handed over to him. As part of CMHT work the post-holder will
also be able to attend GP liaison meetings.
The Manchester Homeless Team is a city-wide community mental health
team specifically dedicated to serving the needs of homeless people.
The team’s clinical base is located on Oxford Road, opposite
the main MRI site. The post-holder will have the opportunity of
working with the team in assessing mental health needs of homeless
people referred by a variety of statutory and voluntary sector agencies
located mainly around Manchester city centre. The team carries a
small caseload of homeless people with severe and enduring mental
illness for enhanced CPA case management. The post-holder will be
able to participate in mental health reviews and will be able to
attend the weekly team meeting together with Dr. Jefferson. There
is a Staff Grade doctor working with this team and Dr Jefferson
also offers supervision for a special interest session with the
Homeless Team.
CLINICAL WORK:
The post holder will have two Special interest sessions and two
Research sessions. It will be possible for these to be time-tabled
flexibly on non-clinical days (Monday, Tuesdays).
The post-holder will be responsible for a new-patient clinic and
a follow-up clinic at the Rawnsley Building. CMHT clients seen in
these clinics will be discussed in supervision with Dr Jefferson.
The post-holder will help provide senior medical input to West
Sector CRHT. This will involve attending the CRHT Team Meeting (Friday
1000-1200) and a brief mid-week medical review (‘board round’)
(Wednesdays, time flexible).
The post holder will also attend the Homeless Team Meeting on Wednesday
(1100-1200). There will not be an expectation to undertake routine
reviews for the Homeless Team, as these are conducted by the team
Staff Grade doctor.
The post holder will have a fixed time for clinical and education
supervision (minimum 1 hour) on a weekly basis with Dr Jefferson.
Supervision will be at Dr Jefferson’s office, Rawnsley Building.
This post is not a core placement.
ON CALL COMMITMENT AND PAY BANDING
1 in 17 full shift, non-resident, rota pattern for out of hours
working operating across the Trust area, thus providing psychiatry
cover to North Manchester General Hospital, Manchester Royal Infirmary
and Wythenshawe Hospital. Out of hours, each hospital site has a
resident ST1-3 trainee and a local on call consultant. The post-holder
will be able to either use their own transport or there will be
taxi provision available to travel around the city. During normal
working hours there is a separate single site rota in operation
at Manchester Royal Infirmary, in which the 8 local ST4-6 higher
trainees participate in to provide middle tier support between the
on call ST1-3 and the on call consultant.

TEACHING / ACADEMIC EXPERIENCE:
The post-holder will be actively involved in the teaching of medical
students and the in the organisation of their clinical attachment.
There will be the opportunity to supervise Problem Based Learning
(PBL) groups.
ACADEMIC ACTIVITES:
The post-holder will have easy access to the extensive academic
program that takes place at the Rawnsley Building, and nearby at
the University of Manchester. This includes a wide range of talks,
clinical meetings and conferences, and often provides the opportunity
to listen to speakers of international standing.
The post-holder will be able to participate in the local Central
Manchester site academic programme. This includes a weekly clinical
Case-Conference and Journal Club.
The Rawnsley Building is the primary location for lectures on the
Speciality Teaching Program.
The post-holder will be able to attend Manchester Medical Society
division of Psychiatry meetings which are usually held nearby at
the University.
FACILITIES:
The trainee will have an office at the Rawnsley Building. This
office may be shared with another ST4-6 trainee. The office has
a PC with net access. Secretarial support is provided by Dr Jefferson’s
secretary.
SUPERVISION ARRANGEMENTS:
The post-holder will have (minimum 1 hour) clinical and educational
supervision on a weekly basis with Dr Jefferson (see time-table).
AUDIT/RESEARCH OPPORTUNITIES:
There are extensive research opportunities, with close links to
the main University of Manchester Academic Psychiatry Unit.
Manchester Mental Health Social Care (MMHSC) Trust has an established
audit programme that the trainee will be able to join.
MANAGEMENT TRAINING:
The post-holder will have the opportunity to become involved in
the management and service development of Central CRHT, West Sector
CMHT and the Homeless Team.
