1. DETAILS OF PLACEMENT (LOCATION, POPULATION, INPATIENT
OR OUTPATIENT, BEDS)
This post is a liaison psychiatry placement within the Psychological
Medicine Service based at Royal Oldham Hospital. The department
is currently expanding and has two consultants (one part time),
2 clinical psychologists, 1 CBT therapist and space for up to two
clinical psychology trainees. There are also 3 administrative and
clerical staff. The team works closely with specialist physiotherapists
(chronic pain), clinical nurse specialists, GP’s with a special
interest and physicians.
The service is both outpatient and community focussed, with two
strands. The first is based at the Royal Oldham Hospital and is
open to all referrals under 65 from Acute Trust physicians and the
breast cancer team. The trainee will run two such clinics a week
seeing a blend of new and follow up patients. The second strand
is off site and part of the local Pennine Musculoskeletal Partnership
NHS ICAT service which is concerned with people suffering from musculoskeletal
problems.
There is a wide variety of patients ranging from people with adjustment
disorders and PTSD, for example after accidents or diagnosis of
cancer, to people struggling to manage their physical condition,
for example pain or diabetes. Patient may also be referred with
medically unexplained symptoms/ somatoform disorders. Many patients
have depression and anxiety in the context of physical illness
The department has strong links with both secondary care mental
health services and the local clinical psychology service and is
very supportive of and sensitive to the trainees needs.
Educational supervision is provided by Dr Burlinson who works part
time, but additional clinical supervision can be given by Dr Singhal.

2. DETAILS OF ROLE AND RESPONSIBILITIES AND EXPERIENCE
TRAINEE WILL GAIN
Outpatient Work: The Psychological Medicine Service is both outpatient
and community focussed, with two strands. The first is based at
the Royal Oldham Hospital and is open to all referrals under 65
from physicians and the breast cancer team. The trainee will run
two such clinics as week seeing a blend of new and follow up patients.
The second strand is off site and part of the local ICATS service
which is concerned with people suffering musculoskeletal problems.
This creates an opportunity for trainees to experience delivering
services within one of the new service delivery frameworks in the
NHS; an Integrated Clinical Assessment and Treatment Service
Allocation Meeting: There is a weekly MDT which discusses referrals
from both parts of the service, in house referrals (e.g. for therapy)
and difficult case discussion.
Community Work: There is no community work except when the patient
is unable to attend a clinic or as part of on call duties.
Inpatient Work: There is no inpatient work apart from as part of
on call duties. However the trainee is encouraged to pilot a new
service as part of their service development experience, and many
have done this in inpatient areas e.g. stroke, respiratory, haematology.
Medico legal work: There is no direct medico legal work in the
placement.
Any other clinical commitment: Trainees are expected to set up
and run a small pilot liaison service with the acute hospital as
described under IP work above. Other areas that have been developed
include Parkinson’s disease, antenatal and pain.
3. JUNIOR DOCTOR SUPPORT AND SUPERVISION
There are currently no junior doctors in the team, but there are
opportunities to get involved with supervision and support of the
junior doctors from other psychiatric and medical teams, for example
doing WPBA. This may change as the other consultant in the team
is bidding for a foundation doctor, a bid that the existing trainee
has been involved in developing.
4. ON CALL COMMITMENT AND PAY BANDING
Daytime on-call: No day time commitments
Out of hours on-call: Covering urgent assessments in Oldham area
(mainly Section 135/136 and MHA assessments) and providing support
for on-call junior doctor and liaison team at Royal Oldham. Approximately
1 in 8 frequency, but only 3 middle grades.
Banding: 1B
5. TEACHING / SUPERVISION / ACADEMIC EXPERIENCE
There are many opportunities for teaching in this post. This includes
co-ordinating the delivery of the mental health aspect of the Foundation
Curriculum to FY1 + FY2 trainees based at the ROH. There are also
opportunities to teach both mental health staff and general hospital
staff, including non medical staff.
6. AUDIT / RESEARCH OPPORTUNITIES
The educational supervisor will support research in the department
and audit is an expected part of the attachment. There is service
outcome data which is collected routinely by the liaison service
and could be accessed for analysis.
There is allocated time in the weekly schedule, and encouragement,
to pursue research.
7. MANAGEMENT EXPERIENCE
The trust has generally been supportive of people wanting to get
involved in management, and is receptive to juniors wanting to get
involved in trust-level projects. The educational supervisor is
happy for the trainee to attend various management and service development
meetings with her. The trust is also supportive of this as well
as providing opportunities to attending coroners inquests with the
trust solicitor and serious untoward incident reviews
As previously mentioned, there is opportunity to develop own pilot
project and lead the foundation psychiatry programme.

8. ADDITIONAL EXPERIENCE AVAILABLE
There is an opportunity to be supervised to do CBT with selected
patients. Depending on trainee preference, there is also the possibility
to conduct joint assessments with clinical psychologists and other
multi-disciplinary members of the ICATS team such as physiotherapists..
The supervisor is very supportive of taking up opportunities both
within liaison, for example spending time with an inpatient liaison
service, or within a medical service e.g. attending rheumatology
clinics or the CFS service.
The trainee is expected to identify an area which would benefit
from psychiatric input and to liaise with them and set up a pilot
project, for example with the local hospice or antenatal service.
This affords management experience in additional to specific clinical
work.
