Description of placement
Trafford is a district of Manchester, with both inner-city
and suburban elements. Psychiatric Services are provided by the Bolton,
Salford and Trafford Mental Health Trust, a specialist mental health
NHS Trust. Services within Trafford are sectorised. The trainee works
with the Central CMHT, which caters to the population of Sale, a defined
urban area, with a population of about 34,000 adults of working age.
Trafford has been commissioned to provide a service to those adults
with severe and enduring mental illness. Other providers have been
commissioned to provide services to people with drug and alcohol
problems and to people with non-psychotic disorders presenting in
primary care settings.
There are four Consultants dealing with patients of working age,
one of whom, Dr Amin, also being approved as an SpR trainer. There
are three consultants dealing with the elderly (with SpR), a part
time post in drug addiction and a part-time post in Rehabilitation
/ Assertive Outreach.
The four general adult psychiatrists each work with both the inpatients
and out-patients from their sector. Collaboration with the CMHTs
has always been good; over recent months, there have been clear
moves towards commonality of case loads and a co-ordinated approach
to patients presenting in emergencies. The inpatient unit has many
strengths and there is good multi-disciplinary working. Effective
bed management and close collaboration between in-patient and community
staff helps to maintain throughput on the in-patient unit. There
is an A+E liaison team, together with an Intensive Support Team
that provides high levels of input to patients at times of crisis
and which may, at times, avoid inpatient admission.
The Consultants are on-call on a 1 in 8 rota. Other than those
times when the SpR is also on call, there is no intermediate tier.
The SpR is expected to become fully involved in the work of the
team. Clinics are carried out in community settings, where there
are opportunities for interaction with GPs. The SpR works closely
with the CMHT in assessing new patients and in providing clinical
supervision to more junior doctors, such as the SHO and Hon Clinical
Assistant.
In addition to clinical responsibilities, the Trainer also has
management and educational responsibilities, as Clinical Tutor and
Clinical Director. This provides the opportunity for the Trainee,
if interested, to take a significant role in particular projects,
such as in Medicines Management / Care Pathway Development or in
Management of the Directorate. These projects provide valuable opportunities
for the trainee to work with senior managers in the Trust Headquarters
and in the PCT.
